For Each Dog Weighing less than 20lbs (pounds), no other pets allowed.  Will you be bringing a pet? 
Pets are allowed, two small dogs under 20 pounds only.  No other pets allowed of any kind.  A pet deposit of $100 is charged and a pet fee for each dog under 20 pounds.  How many dogs under 20 pounds will you be bringing?

I also understand at the option of Supercabins, W.A.S. Properties, LLC, or Walden’s Creek, LLC, that this reservation is not binding on Supercabins, W.A.S. Properties, LLC or Walden’s Creek LLC until I have done all of the following: Signed a completed reservation agreement and returned it to Supercabins, submitted a photocopy of the front and back of my driver’s license, submitted a photocopy of the front and back of my credit card that I made the charge with, and my reservation agreement has been signed by either W.A.S. Properties or Walden’s Creek, LLC and I have received a written confirmation number.
